A comprehensive cascade of care for people living with HIV/AIDS in Kenya
First step in our healthcare cascade: identifying and preventing new infections
Multiple testing strategies including family testing, walk-in services, PITC, house-to-house testing, and community outreach by qualified counselors.
Comprehensive diagnostic testing including CD4 count, viral load, hematology, biochemistry, and other essential tests for monitoring HIV progression and treatment efficacy.
Daily health talks by staff and CHVs on hygiene, nutrition, OIs, prevention, child protection, and support groups led by peer educators.
Second step: Comprehensive medical treatment and ongoing clinical management
Provision of triple therapy HAART in line with National ART Guidelines for all enrolled HIV clients to ensure effective viral suppression.
Screening all clients for TB at every visit, providing treatment per National Guidelines, and Isoniazid preventive therapy for PLHIV without TB.
Third step: Supporting overall wellbeing and quality of life
Nutritional screening, counselling and support based on BMI <18.5 for adults, children <12 years, pregnant/breastfeeding mothers, HIV/TB co-infected, and elderly.
Comprehensive mental health support including individual counselling, support groups, trauma therapy, and crisis intervention for patients and their families affected by HIV/AIDS.
Community visits by staff and CHVs for patients too ill to visit centers, offering personal care, medical evaluation, and newborn/maternal follow-up.
Fourth step: Targeted interventions for specific populations
Elimination of Mother to Child Transmission through triple ART therapy during pregnancy, continued for life to reduce vertical transmission risk.
Antenatal care, weight monitoring, immunization for children under five, integrated services at one point to minimize missed opportunities.
Screening almost 100% of women of reproductive age since 2013, early detection and treatment of cervical cancer at various stages.
Each step builds upon the previous to provide comprehensive care
